Walker Healthforce is seeking a Case Manager I – LTSS with at least 2 years of health care experience to support care management and care coordination for members with high-need potential, including long-term services and supports (LTSS). This is a Contract opportunity.
Start Date: ASAP
Location: Remote – Rockford, Illinois; 25-40% estimated local travel may be required
Hours/Schedule: Monday-Friday, 8:00 AM-5:00 PM CT, 40 hours; 25-40% estimated local travel may be required
Worker Type: W2
Core Requirements
At least 2 years of health care experience.
At least 1 year working with persons with disabilities/chronic conditions and long-term services and supports (LTSS).
At least 1 year of care management experience, or experience in a medical and/or behavioral health setting, or an equivalent combination of relevant education and experience.
Knowledge of community resources and experience coordinating services across diverse member needs and settings.
Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel requirements, unless otherwise required by law.
Strong communication, problem-solving, critical-thinking, time-management, prioritization, and independent-work skills.
Microsoft Office proficiency and ability to navigate online portals and databases.

Job Responsibilities
Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
Support waiver enrollment/disenrollment and develop care plans and waiver service plans with members, caregivers, providers, and support networks.
Monitor care plans, document interventions and goal progress, and recommend changes as appropriate.
Coordinate behavioral health, LTSS, and home- and community-based services to support continuity of care.
Assess medical necessity, authorize appropriate waiver services, and evaluate covered benefits and funding sources.
Facilitate interdisciplinary care team collaboration and coordinate with licensed care managers and leadership as needed.
Identify barriers and critical incidents and develop appropriate coordination and prevention plans.
